About The Position

Blended case management is a service which will assist eligible individuals with mental illness, including children with a serious mental illness or emotional disorder, in gaining access to needed medical, social, educational, and other services. The role involves assisting consumers or their families in obtaining and maintaining culturally appropriate basic living needs and skills such as housing, food, medical care, recreation, education, and employment. Services are provided within the context of the consumer and their family's culture, and in accordance with a written, consumer-specific service plan that assesses strengths, needs, and interests, and is goal and outcome-oriented. Case management services are provided as needed in the place where the consumer/family resides or needs the service, or in the Case Management office when off-site interventions are not appropriate. Face-to-face contact frequency is determined by assessment, ranging from once a week to once a month. The role includes providing crisis intervention services to all children and adolescent Case Management consumers, completing required paperwork by stated deadlines, maintaining Case Management records, and ensuring continuity of care for hospitalized children and adolescents. Telehealth services will also be provided when necessary.

Requirements

  • A bachelor’s degree with major course work in sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, theology, counseling, education.
  • Minimum of one (1) year experience (paid or unpaid) in a human service field. This experience must involve direct contact with the individual receiving services (i.e. coaching, teaching, case management, etc.)
  • (2) years paid experience in public or private human services with one (1) year in direct consumer contact.
  • All Case Managers must also show proof of a valid driver's license and current insurance
  • Must have a registered operable Automobile.

Responsibilities

  • Assist the consumer or their family in obtaining and maintaining culturally appropriate basic living needs and skills such as housing, food, medical care, recreation, education and employment.
  • Provide services within the context of the consumer and their family's culture.
  • Provide case management service in accordance with a written, consumer-specific service plan which assesses strengths, needs interests and is goal and outcome oriented.
  • Provide case management services as needed in the place where the consumer/family resides or needs the service. Services may also be provided in the Case Management office when off-site interventions would not be appropriate.
  • Contact the consumer or their family will be determined by an assessment of their need. Face-to-face contact with the consumer shall be made on the basis of the assessment and will range from once a week to one time per month.
  • Ensure that when a consumer or their family that cannot be contacted during any visit that they are provided immediate follow-up and that this continues until the consumer's status is clarified.
  • Provide crisis intervention services to all children and adolescent Case Management consumers.
  • Written case notes should document the purpose for each contact and reflect the goals and objectives of the service plan.
  • Complete all required paperwork and submit such paperwork by stated deadlines.
  • Maintain Case Management records and ensure that all documentation is included in the consumer’s records.
  • Provide continuity of care for children and adolescents receiving Case Management services who are hospitalized.
  • Provide telehealth services when necessary to the consumer and their family or any other pertaining parties that is part of the consumer’s case.
