About The Position

Wider Circle delivers community-rooted, relationship-driven care for older adults and clinically complex populations by aligning clinical insight with trusted, community-based support. Wider Care+ is our new pilot that pairs physicians, Community Health Workers, Licensed Clinical Social Workers, and care navigators to create a coordinated, humane path forward. You are the clinical entry point—the comprehensive assessment that sets the foundation for real, ongoing intervention. A Shared Medical Appointment (SMA) is a group-based medical care model that allows multiple patients with similar conditions to receive education, support, and medical guidance in a structured setting. Patients benefit from additional time with healthcare professionals, peer support, and ongoing engagement in their care. The SMA model combines individual medical decision-making with group education and discussion. The physician has two primary responsibilities: Initial Intake Appointment (Individual Visit): Prior to enrollment in an SMA group, each patient completes an individual intake appointment with the physician to review medical history, assess symptoms, determine clinical appropriateness, establish a care plan, and document eligibility. Round-Robin Group Check-Ins: During recurring SMA sessions, the physician conducts brief medical check-ins, reviews progress, addresses questions, reinforces care plans, and provides medical oversight while creating an E&M charting note per patient.

Requirements

  • MD or DO with active, unrestricted Arizona license. Multiple state licenses strongly preferred.
  • Substantial experience with Medicare/Medicaid populations, or complex chronic disease management
  • Skilled at telehealth assessment and building rapport virtually
  • Strong clinical judgment, comfort with complexity and ambiguity, and interest in care coordination
  • Comfortable working in an iterative, startup-like pilot environment

Nice To Haves

  • Board-certified or board-eligible in Family Medicine, Internal Medicine, Geriatrics, or related primary care specialty preferred

Responsibilities

  • Conduct comprehensive telehealth initial visits for enrolled members (15-20 min)
  • Synthesize complex medical and social histories and identify clinical priorities and risks
  • Create concise, actionable clinical plans and handoffs for Community Health Workers, LCSWs, and care navigators
  • Document findings and recommendations clearly in the EMR to guide ongoing care
  • Provide clinical guidance on escalation pathways and program workflows during the pilot
  • Build rapid clinical rapport and trust with members and caregivers
