About The Position

Our client is seeking a motivated, compassionate, and highly organized Bilingual Primary Care Case Manager fluent in Spanish and English. The role aims to improve patient outcomes and engagement while reducing unnecessary emergency room visits and hospital utilization. The Case Manager proactively reviews medical records, identifies patients needing follow-up, contacts patients, and coordinates timely primary care appointments, either in person or through telehealth. This is a non-licensed care coordination position unless otherwise specified. The Case Manager does not independently diagnose, provide medical advice, perform clinical triage, or modify treatment plans. Clinical decisions and medical concerns must be referred to an appropriate licensed healthcare professional. The schedule is Monday through Friday, 8:00 AM–5:00 PM PST, with a one-hour unpaid break, for 40 work hours per week. The posting describes the role as an independent contractor position and offers permanent work from home.
