About The Position

Chapel Haven is seeking a Benefits Coordinator to oversee the day-to-day management of a caseload of 50–100 community members who subscribe to our Benefits Maintenance Service. The Benefits Coordinator will work closely with families, colleagues, and various federal, state, and local agencies—including the Social Security Administration, Connecticut Department of Social Services, Department of Developmental Services, and Department of Housing Section 8—to process and maintain benefits. This role requires strong attention to detail, compliance with reporting deadlines, and effective communication to ensure the seamless continuation of benefits for the individuals we serve. Requirements

  • Valid CT driver’s license
  • Experience and benefits coordination, case management, or related field
  • Strong organization and communication skills
  • Ability to navigate federal, state, and local benefits programs
  • Proficiency in maintaining accurate records in meeting strict deadlines
  • First Aid, AED & CPR Certification
  • CPI Non-Violent Crisis Intervention Certification
  • Ability to sit, stand and drive for periods of time
  • Bachelor’s degree in Social Work, Human Services, or a related field preferred; equivalent experience in benefits coordination, case management, or processing benefit redeterminations may be considered.
  • Experience working with individuals with intellectual, developmental, or social disabilities preferred.
  • Experience in benefits coordination, case management, or a related field.

Responsibilities

  • Manage a caseload of 50–100 adults receiving benefits services.
  • Maintain active files and case notes documenting account activity.
  • Serve as the Chapel Haven liaison with Connecticut DSS to apply for Medicaid, SNAP, and other entitlements.
  • Work with the Social Security Administration (SSA) to compile and submit monthly wage reports while ensuring adherence to income guidelines.
  • Review and file correspondence appropriately within one week of receipt.
  • Prepare necessary paperwork for benefit changes and submit required reports on time.
  • Ensure compliance with all federal, state, and local reporting deadlines to prevent disruption of benefits.
  • Promote the Benefits Office and provide educational information to families while adhering to guidelines and avoiding direct recommendations or guarantees regarding benefits eligibility.
  • Host online and in-person benefit information sessions, attend planning meetings, and refer families to appropriate resources when necessary.
  • Host occasional after-hours benefit information sessions, not to exceed four (4) sessions per year.
  • Accompany individuals to DSS, DDS, or SSA appointments to assist with resolving issues and submitting required paperwork.
  • Schedule and attend hearings as required.
  • Participate in SLP department meetings and other meetings as assigned by the supervisor.
