About The Position

Oak Street Health’s graduate Behavioral Health Specialist Fellowship is a 12-month paid training program for social work graduates that prepares fellows to provide behavioral health care to older adults. The program focuses on the collaborative care model in a value-based setting and includes training in evidence-based interventions and psychotropic medications, clinical supervision hours from a licensed clinical social worker, and didactic learning designed to build on graduate training.

Fellows are expected to work at Oak Street Health for 24 months after completing the fellowship. The role is based at the Rocket City location in Huntsville, Alabama. Oak Street Health provides personalized primary care for older adults on Medicare, with an emphasis on quality of care and improving community health outcomes.

Requirements

• Graduate or impending graduate of a Family, Adult, or Adult-Gerontology Social Work training program.
• Minimum GPA of 3.0 in the MSW program.
• Hold, or be able to hold by the fellowship start date, an active, non-probationary lower-level social work license in the state of employment.
• Willingness to work in the assigned location for the duration of the program.
• Genuine passion for behavioral health.
• Supportive attitude toward older adults.
• Embrace teamwork and collaboration with colleagues.
• Interest in an innovative, empirically guided population health model.

Responsibilities
