About The Position

Amira Choice of Roseville on Lexington Avenue is a senior living community in Roseville, Minnesota, offering independent living, assisted living, memory care, and care suites. Managed by Ebenezer, the community has 80 senior living units and is near public transportation. The Assistant Director of Health Services is an entry-level nursing leadership position for a registered nurse, supported by the Director of Health Services, a regional nurse, a triage nurse, and resident assistants. The role oversees resident assessments, service agreements, care coordination, clinical documentation, and communication about residents’ health needs in accordance with applicable Minnesota regulations and the Nurse Practice Act.

Requirements

Vocational school/technical training or an associate degree in nursing.
One to three years of experience.
Minnesota Registered Nurse (RN) license.
Basic Life Support certification from the American Heart Association or Red Cross.

Nice To Haves

Bachelor of Science in Nursing.
Three to five years of experience.
Prior supervisory experience.
Prior experience in home care or an assisted living environment.

Responsibilities

Maintain current assessments and documentation of clients’ health status.
Perform initial and ongoing assessments in accordance with applicable Minnesota Assisted Living regulations and the Nurse Practice Act, including RN assessments upon admission, within 14 days of admission, at least every 90 days, and when a health condition changes.
Collaborate with clients and/or responsible parties through education and timely communication to support appropriate services and placement.
Ensure clients have current service agreements; implement agreements for new residents and review and update them at least annually or more often when service needs change.
Develop, maintain, and implement individual emergency plans based on health status and advance care planning documents, including POLST.
Maintain clinical charts and progress notes using a computerized documentation system.
Communicate changes in client conditions to medical professionals and other relevant individuals.
Coordinate client health services and facilitate coordination with internal and external providers.
Ensure physician orders are signed and implemented in accordance with applicable Minnesota Assisted Living regulations.
Work with clients and/or their representatives to ensure appropriate service delivery and funding.
Participate in care conferences as needed or requested by the Director of Health Services or Executive Director.
