The APRN provides exceptional primary medical care to patients ages 65 and older, including patients with acute illness and complex chronic health needs. Under physician supervision, the APRN conducts physical exams, orders and interprets diagnostic tests, develops treatment plans, and assesses, diagnoses, treats, manages, educates, and coordinates care using evidence-based medicine and practice guidelines. The role is safety-sensitive because it involves direct patient care and medication administration. The APRN is accountable for patient care and outcomes, consults specialists when appropriate, stays involved in patients’ detailed care, engages with hospitalists when patients are hospitalized, and ensures patients are seen by a physician every other visit. The role also requires complying with clinical, regulatory, safety, and internal requirements; maintaining timely and complete electronic medical records; protecting patient confidentiality; and completing operational and documentation tasks. The APRN reports to the Regional Manager (or a similar role) and works closely with physicians and other healthcare professionals.
