About The Position

This role supports older adults by providing information, counseling, outreach services, and senior center assistance throughout Wayne County. You will work directly with the community to ensure that individuals receive guidance, resources, and helpful support. As an Aging Case Aide 2, you will perform the following duties: Program Guidance: Provide information and benefits counseling on programs such as Medicare, PA MEDI, PACE, LIHEAP, and Tax/Rent Rebate Application Support: Assist older adults with completing applications and accessing community referrals Outreach Services: Conduct outreach across Wayne County, including home visits to homebound individuals Senior Center Coordination: Organize programs, activities, and special events at senior centers, and supervise volunteers Nutrition Support: Assist with meal distribution, sanitation practices, and donation documentation in daily nutrition services Record Management: Maintain accurate records and perform data entry in agency and statewide databases.

Requirements

  • One year as an Aging Case Aide 1; or Two years of public contact work, including one year in a human services program; or Any equivalent combination of experience and training.
  • Meet the PA residency requirement.
  • Able to perform essential job functions.
  • Pass a background investigation.
  • Pass a drug screening (conditional offer of employment).
  • Satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance (conditional offer of employment).
  • Attach a copy of college transcripts if claiming education.
