About The Position

The Monroe County Area Agency on Aging is seeking an Aging Care Manager 3 who is passionate about helping the older Pennsylvania population. Requirements

  • One year as an Aging Care Manager 2 or Aging Program Assessor; or
  • One and one-half years of professional human services, law enforcement, or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
  • An equivalent combination of experience and training which includes six months of professional human services, law enforcement, or nursing experience.
  • Meet the PA residency requirement.
  • Be able to perform essential job functions.
  • Satisfactory criminal history reports, including PA State Police and FBI clearance (conditional offer).
  • Satisfactory criminal history reports, including PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance (conditional offer).

  • Possibility of some work from home hours after a probationary period.

Responsibilities

  • Review categories of need and begin investigations within required time frames according to regulations of the Older Adults Protective Services Act.
  • Reduce or mitigate risk to older adults and complete investigations within regulated timelines.
  • Develop care plans, complete assessments, and document contacts using the SAMS program.
  • Arrange medical, psychological, and social assessments and connect individuals with appropriate services.
  • Provide education on protective services and mandatory reporting through public presentations.
  • Meet with supervisors, attend network meetings, and assist with related task force activities.
