About The Position

The Aging Care Manager position offers a rewarding opportunity to make a meaningful difference in the lives of older adults. This position provides comprehensive assessment, care coordination, advocacy, and supportive services to older adults who may be unable to fully meet their own needs. The Aging Care Manager works to ensure individuals receive the appropriate resources and services necessary to maintain their independence, dignity, safety, and quality of life while remaining in their homes and communities whenever possible. Requirements

  • One year as an Aging Care Manager 2 or Aging Program Assessor; or One and one-half years of professional human services, law enforcement, or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or An equivalent combination of experience and training which includes six months of professional human services, law enforcement, or nursing experience.
  • Must meet the PA residency requirement.
  • Must be able to perform essential job functions.
  • A conditional offer of employment will require a medical exam and a drug screening.
  • A conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.

Responsibilities

  • Conduct investigations of alleged abuse, exploitation, or neglect
  • Evaluate medical, social, and psychological needs to determine risk levels
  • Implement measures that safeguard vulnerable older adults
  • Recommend court interventions, including guardianships and involuntary commitments
  • Document all actions, findings, and service plans
  • Coordinate plans with partnering agencies to refer individuals for additional support
