DESCRIPTION OF WORK This position focuses on assessing needs, coordinating services, and supporting older adults and eligible individuals through clear guidance and responsible care management. Work involves maintaining cases, completing assessments, and assisting consumers with information about available programs.
Requirements
- Six months as an Aging Care Manager 1 or a County Caseworker 1; or Successful completion of the County Social Casework Intern program; or Six months of professional human services or nursing experience; and a bachelor’s degree in social sciences, behavioral sciences, human services, or a closely related field; or An equivalent combination of experience and training.
- Meet the PA residency requirement.
- Be able to perform essential job functions.
- Pass a background investigation.
- Pass a drug screening (conditional offer of employment).
- Satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance (conditional offer of employment).
Responsibilities
- Complete intake and reassessment duties and document information in the appropriate systems
- Arrange needed services and work with community partners to support consumer needs
- Explain program eligibility, limits, and procedures to consumers and families
- Manage caseload tasks such as reports, records, and required follow-up contact
- Interpret requests from professionals and determine appropriate referrals
