Aging Care Manager 3 (Local Government) - Delaware County Area Agency on Aging

Requirements
- One year as an Aging Care Manager 2 or Aging Program Assessor; or One and one-half years of professional human services, law enforcement, or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or An equivalent combination of experience and training which includes six months of professional human services, law enforcement, or nursing experience.
- Possession of a valid driver’s license which is not under suspension.
- Meet the PA residency requirement.
- Ability to perform essential job functions.
- Pass a background investigation.
- Satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance.
- Satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
Responsibilities
- Gather information from consumers, caregivers, and others to determine the level of risk and whether reports are substantiated
- Develop plans that address concerns, arrange supports, and coordinate needed services to reduce risks
- Maintain detailed records of contacts, observations, and case activity in the required system
- Conduct face to face visits to monitor safety, review concerns, and speak with individuals involved in the case
- Arrange shelter, initiate involuntary procedures, or seek guardianship when necessary to protect consumers
- Prepare documents for court proceedings and provide testimony when required
