About The Position

Monroe County Area Agency on Aging is searching for an Aging Care Manager 3 to work with individuals, families, and community partners to ensure safety and well-being. This position allows you to build strong relationships while responding to important needs in the community, offering support, guidance, and coordination for essential services. Requirements

  • One year as an Aging Care Manager 2 or Aging Program Assessor; or One and one-half years of professional human services, law enforcement, or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or An equivalent combination of experience and training which includes six months of professional human services, law enforcement, or nursing experience.
  • Meet the PA residency requirement.
  • Be able to perform essential job functions.
  • Satisfactory criminal history reports, including PA State Police and FBI clearance.
  • Satisfactory criminal history reports, including PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.

  • Telework: You may have the opportunity to work from home (telework) part-time, after six months of employment. In order to telework, you must have a securely configured high-speed internet connection and work from an approved location inside Pennsylvania.

Responsibilities

  • Receive and evaluate reports of need in accordance with the Older Adult Protective Services Act
  • Begin investigations promptly, complete required steps, and determine the level of risk to the older adult
  • Develop care plans, arrange assessments, and document case details in the SAMS system
  • Provide outreach and public education on protective services and mandatory reporting requirements
  • Manage complex cases, coordinate resources, and maintain strong working relationships to reduce risk
  • Provide 24 hour emergency services as scheduled and support the Elder Abuse Task Force
