About The Position

Helms Home Care is seeking a full-time Afternoon/Evening Coordinator to support patients, families, nurses, pharmacies, and physicians in coordinating home infusion services. The role coordinates referrals and visits, maintains accurate patient information, and helps ensure clear communication across the care team. The position is based on-site in Denver, North Carolina, Monday through Friday, from 12:30 PM to 9:30 PM, with a listed salary range of $20.00–$22.00. The successful candidate is dependable, self-motivated, organized, and resourceful, and can take initiative and manage day-to-day responsibilities independently with minimal supervision. They should also recognize when concerns require escalation to the appropriate team members.

Responsibilities

Coordinate patient referrals, nurse assignments, and visit schedules.
Communicate professionally with patients, families, nurses, pharmacies, and physicians.
Review referrals and ensure required documentation and information are complete.
Match nurses to patients based on location, therapy, skills, and patient-specific needs.
Verify required nurse training and coordinate training when needed.
Maintain accurate scheduling, patient, and care coordination records.
Document communications, concerns, and patient updates promptly.
Coordinate required labs and follow up to ensure results are received appropriately.
Communicate changes in patient care plans to nursing staff.
Identify and resolve scheduling and coordination issues independently when appropriate.
Recognize concerns that require additional support and escalate them to the appropriate clinical team members.
Maintain patient confidentiality and follow HIPAA and company standards.
Manage daily responsibilities independently while maintaining communication with the broader team.
Participate in team meetings and process improvement efforts.
