About The Position

The Advanced Practice Provider (APP) provides medical services to individuals, families, and groups, emphasizing health promotion and disease prevention. The role includes assessment, diagnosis, and management of acute and chronic diseases, with duties performed in accordance with the mission, vision, and values of Tampa General Hospital.

This position provides longitudinal outpatient management of patients with COPD, including new and follow-up visits, post-hospital care, medication and inhaler optimization, oxygen management, review of diagnostic testing, patient education, and coordination with pulmonary rehabilitation and other services. The role is designed to improve access to pulmonary care, optimize COPD management, identify clinical deterioration early, and reduce preventable exacerbations, emergency department utilization, and hospital readmissions. The position is for an Advanced Practice Registered Nurse or Physician Assistant.

• Must be an Advanced Practice Registered Nurse or Physician Assistant.

Responsibilities

• Provide longitudinal outpatient management of patients with COPD, including new and follow-up visits.
• Provide post-hospital care.
• Optimize medications and inhaler use.
• Manage oxygen needs.
• Review diagnostic testing.
• Educate patients.
• Coordinate with pulmonary rehabilitation and other services.
• Assess, diagnose, and manage acute and chronic diseases.
• Promote health and disease prevention.
