Advanced Practice Provider - Advanced Lung Disease- Pulmonary HTN
About The Position
The Advanced Practice Provider (APP) provides medical services to individuals, families, and groups, emphasizing health promotion and disease prevention. The role includes assessment, diagnosis, and management of acute and chronic diseases, with job duties performed in accordance with the mission, vision, and values of Tampa General Hospital. The APP provides specialized longitudinal care for patients with pulmonary hypertension (PH) and chronic thromboembolic pulmonary hypertension (CTEPH). This includes diagnostic evaluation, management and monitoring of PH-directed therapies, patient education, and multidisciplinary care coordination. The position is currently clinic-based and is designed to evolve with program growth to support an admitting inpatient PH/CTEPH service. The APP will provide continuity across ambulatory and inpatient settings while supporting expansion of the CTEPH program, development of standardized clinical pathways, and care of an increasingly complex pulmonary vascular patient population.
Requirements
- Advanced Practice Registered Nurse with Acute Care Certification, or Physician Assistant.
- Three years of related experience.
Responsibilities
- Provide assessment, diagnosis, and management of acute and chronic diseases.
- Provide longitudinal care for patients with pulmonary hypertension and CTEPH.
- Conduct diagnostic evaluation and manage and monitor PH-directed therapies.
- Educate patients.
- Coordinate multidisciplinary care.
- Provide continuity across ambulatory and inpatient settings as the program develops.
- Support expansion of the CTEPH program and development of standardized clinical pathways.
