Advanced Practice Provider - Advanced Lung Disease- COPD
About The Position
The Advanced Practice Provider (APP) provides medical services to individuals, families, and groups, emphasizing health promotion and disease prevention. This includes the assessment, diagnosis, and management of acute and chronic diseases. The APP performs job duties in accordance with the mission, vision, and values of Tampa General Hospital. The role provides longitudinal outpatient management of patients with COPD, including new and follow-up visits, post-hospital care, medication and inhaler optimization, oxygen management, review of diagnostic testing, patient education, and coordination with pulmonary rehabilitation and other services. The role is designed to improve access to pulmonary care, optimize COPD management, identify clinical deterioration early, and reduce preventable exacerbations, emergency department utilization, and hospital readmissions.
Requirements
- Must be an Advanced Practice Registered Nurse or Physician Assistant.
Responsibilities
- Provide longitudinal outpatient management for patients with COPD.
- Conduct new and follow-up visits and provide post-hospital care.
- Optimize medications and inhaler use.
- Manage oxygen therapy.
- Review diagnostic testing.
- Educate patients.
- Coordinate care with pulmonary rehabilitation and other services.
- Assess, diagnose, and manage acute and chronic diseases.
- Provide medical services emphasizing health promotion and disease prevention.
- Perform job duties in accordance with Tampa General Hospital’s mission, vision, and values.
