Advanced Practice Provider - Advanced Lung Disease- Pulmonary HTN
About The Position
The Advanced Practice Provider (APP) provides medical services to individuals, families, and groups, emphasizing health promotion and disease prevention. This includes assessing, diagnosing, and managing acute and chronic diseases. The APP is responsible for performing job duties in accordance with the mission, vision, and values of Tampa General Hospital. The role provides specialized longitudinal care for patients with pulmonary hypertension and chronic thromboembolic pulmonary hypertension (CTEPH), including diagnostic evaluation, management and monitoring of pulmonary hypertension-directed therapies, patient education, and multidisciplinary care coordination. The position is currently clinic-based and is designed to evolve with program growth to support an admitting inpatient PH/CTEPH service. The APP will provide continuity across ambulatory and inpatient settings while supporting expansion of the CTEPH program, development of standardized clinical pathways, and management of an increasingly complex pulmonary vascular patient population.
Requirements
- Advanced Practice Registered Nurse with 3 years of related experience.
- Acute Care Certification.
- Physician Assistant.
Responsibilities
- Assess, diagnose, and manage acute and chronic diseases.
- Provide longitudinal care for patients with pulmonary hypertension and CTEPH.
- Conduct diagnostic evaluations and manage and monitor pulmonary hypertension-directed therapies.
- Educate patients and coordinate multidisciplinary care.
- Provide continuity across ambulatory and inpatient settings as the program evolves.
- Support expansion of the CTEPH program and development of standardized clinical pathways.
