About The Position

The Advanced Practice Provider (APP) provides medical services to individuals, families, and groups, emphasizing health promotion and disease prevention. This includes assessment, diagnosis, and management of acute and chronic diseases. The APP is responsible for performing job duties in accordance with the mission, vision, and values of Tampa General Hospital.

The role provides longitudinal outpatient management of patients with COPD, including new and follow-up visits, post-hospital care, medication and inhaler optimization, oxygen management, diagnostic testing review, patient education, and coordination with pulmonary rehabilitation and other services. The role is designed to improve access to pulmonary care, optimize COPD management, identify clinical deterioration early, and reduce preventable exacerbations, emergency department utilization, and hospital readmissions.

Responsibilities

• Provide longitudinal outpatient management of patients with COPD.
• Conduct new and follow-up visits and provide post-hospital care.
• Optimize medications and inhaler use.
• Manage oxygen needs.
• Review diagnostic testing.
• Educate patients.
• Coordinate care with pulmonary rehabilitation and other services.
• Assess, diagnose, and manage acute and chronic diseases.
• Support early identification of clinical deterioration and efforts to reduce preventable exacerbations, emergency department utilization, and hospital readmissions.
