About The Position

The Advanced Practice Provider (APP) provides medical services to individuals, families, and groups, emphasizing health promotion and disease prevention. This includes assessment, diagnosis, and management of acute and chronic diseases. The APP performs job duties in accordance with the mission, vision, and values of Tampa General Hospital. The role provides specialized longitudinal care for patients with pulmonary hypertension (PH) and chronic thromboembolic pulmonary hypertension (CTEPH), including diagnostic evaluation, management and monitoring of PH-directed therapies, patient education, and multidisciplinary care coordination. The position is currently clinic-based and is designed to evolve with program growth to support an admitting inpatient PH/CTEPH service. The APP will provide continuity across ambulatory and inpatient settings while supporting expansion of the CTEPH program, development of standardized clinical pathways, and management of an increasingly complex pulmonary vascular patient population.

Requirements

Advanced Practice Registered Nurse with 3 years of related experience and Acute Care Certification, or Physician Assistant.

Responsibilities

Provide specialized longitudinal care for patients with pulmonary hypertension and CTEPH.
Conduct diagnostic evaluation and manage and monitor PH-directed therapies.
Educate patients.
Coordinate multidisciplinary care.
Provide continuity of care across ambulatory and inpatient settings as the program grows.
Support expansion of the CTEPH program.
Support development of standardized clinical pathways.
Manage an increasingly complex pulmonary vascular patient population.
Provide medical services emphasizing health promotion and disease prevention, including assessment, diagnosis, and management of acute and chronic diseases.
