About The Position

Monogram Health is seeking compassionate and skilled Nurse Practitioners or Physician Assistants to deliver high-quality in-home care to patients living with multiple chronic conditions, including diabetes, hypertension, chronic kidney disease, heart failure, COPD, depression, and other complex health needs. The role works within an integrated multidisciplinary care team spanning clinical care, behavioral health, pharmacy, and administration. Providers establish long-term patient relationships and continuity of care through visits in patients’ homes and via telehealth, within a value-based, patient-centered managed-care model focused on population health. There is no on-call duty, overnight work, or weekend work, including paid company holidays. Visits last 30–90 minutes depending on appointment type and patient needs; schedule responsibilities may change with business needs. The role may also involve care in skilled nursing facilities, assisted living facilities, hospitals, and other affiliated clinical settings, as applicable. The position offers a competitive base salary, competency-based awards, an annual anniversary bonus, additional bonus opportunities, mileage reimbursement, and reimbursement for continuing education expenses.
