About The Position
Prime Healthcare is seeking an experienced and compassionate Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join its Family Practice team at a primary care clinic in Evanston, Illinois. This full-time, day-shift role provides comprehensive outpatient care in a collaborative, community-focused setting.
Requirements
- Be a licensed Nurse Practitioner or Physician Assistant in Illinois.
- For Nurse Practitioners, hold board certification in Family Medicine; Physician Assistants must hold appropriate PA certification.
- Have strong interpersonal and clinical skills.
- Demonstrate a commitment to patient-centered care and health education.
Responsibilities
- Deliver full-spectrum primary care to patients of all ages.
- Manage chronic conditions, perform preventative care, and address acute health concerns.
- Work closely with physicians and clinical staff to ensure coordinated, high-quality care.
- Maintain accurate and timely documentation in the electronic medical record.
