About The Position
HarmonyCares is seeking an Advanced Practice Provider (NP or PA) to provide value-based primary care to a high-risk geriatric patient panel in patients’ homes, Monday through Friday. The role focuses on managing chronic conditions and caring for high-acuity patients, with an emphasis on addressing patient needs rather than visit volume. Clinicians are supported by an interdisciplinary, integrated, physician-driven team. The position offers a $135,000–$155,000 annual salary range, a $10,000 signing package, and a quality-based bonus. The role is described as having no holidays, no hospital rounds, and minimal call responsibilities. Appointment support and a company car are available in select markets. HarmonyCares is a national value-based provider of in-home primary care for people with complex medical needs and difficulty accessing care. Its mission is to provide personalized, quality-based healthcare in patients’ homes. Individual compensation packages depend on factors including skills, experience, and qualifications.
Requirements
- Be licensed as a Nurse Practitioner or Physician Assistant in the state where the HarmonyCares Medical Group office is located.
- Hold or be eligible for a federal DEA license.
- Maintain an active CPR/BLS certification.
- Hold board certification.
- Maintain a valid driver’s license and a good driving record.
- Have directly related experience in home health, geriatrics, and primary care.

- Geriatric training or experience is preferred.
