About The Position
The Adult Community Case Manager joins AMHC’s Machias, Maine team to help adults with long-term psychiatric disabilities build the skills, confidence, and resources needed to live successfully in their communities. The role focuses on empowering individuals to overcome barriers, strengthen life skills, improve quality of life, and build greater stability and independence. The position is full-time, Monday through Friday, with typical business hours of 8:00 AM to 5:00 PM. Local travel within the community is required.
Requirements
- An associate degree or equivalent credits in a Human Services field is required.
- Must be certified at the full MHRT-C or provisional level.
- Must obtain and maintain a Behavioral Health Home (BHH) Rater ID as a condition of employment. Employees who do not currently hold a Rater ID will receive training and support necessary to obtain certification and authorization as a rater.
- Use of a personal vehicle and an active driver’s license is required.

Responsibilities
- Build supportive, therapeutic relationships with clients.
- Help individuals set and achieve personal wellness goals.
- Coordinate services and connect clients with community resources.
- Provide supportive counseling and crisis intervention when needed.
- Collaborate with a multidisciplinary team to support whole-person wellness.
- Facilitate skill-building and psychoeducational groups.
