About The Position

Home & Health Care Management is Northern California’s oldest home health care agency, providing in-home care and case management to older adults, children, and people living with disabilities. The Admissions Registered Nurse performs intake and case management activities for applicants and participants, in their homes and through telehealth, to ensure their needs are met and to arrange appropriate referrals when there are gaps in services. The role includes assessing applicants and developing and implementing person-centered Plans of Treatment for enrollment in the HCBA Waiver, California Community Transitions (CCT), and Assisted Living Waiver (ALW). The nurse collaborates with CCT social workers and HCBA case management staff to support enrollment and continued participation. The position is based in the Sacramento office and serves Sacramento and surrounding counties. Work is approximately 20% direct home visits and travel and 80% telehealth visits, calls, and documentation in the office. This is an in-office role, not a remote position.
