About The Position
BAYADA Home Health Care is seeking an experienced Admissions Registered Nurse (RN) for a full-time role with its UCV office. The nurse provides home health admissions and OASIS Start of Care (SOC) visits to adult and geriatric clients across Monmouth, Middlesex, Union, and Somerset Counties, New Jersey. This dedicated admissions role involves one-on-one care, autonomous fieldwork, flexible scheduling, and collaboration with an interdisciplinary clinical team. The Admissions RN welcomes new clients, completes comprehensive initial assessments, establishes individualized plans of care, and coordinates a smooth transition into home health services. Compensation is per point, with a stated base rate of $65–$75 per point and annual earning potential of $101,400–$117,000, depending on qualifications and other stated factors. A $20,000 sign-on bonus is offered to full-time RNs in Monmouth County only.
Requirements
- Current, unrestricted New Jersey Registered Nurse (RN) license.
- At least one year of recent, verifiable clinical nursing experience.
- Prior home health experience.
- OASIS experience.
- Strong clinical assessment and care coordination skills.
- Excellent communication, interpersonal, organizational, and time-management skills.
- Ability to work independently and manage a field-based schedule effectively.
- Reliable transportation and ability to travel throughout Monmouth, Middlesex, Union, and Somerset Counties.

- Experience with electronic medical records (EMR).
Responsibilities
- Complete comprehensive OASIS Start of Care admissions visits for home health clients in the assigned territory.
- Perform initial nursing assessments, including medication reconciliation, safety assessments, and evaluation of clinical and functional needs.
- Develop individualized plans of care with physicians, clients, caregivers, and interdisciplinary team members.
- Identify client needs and coordinate nursing, therapy, social work, and other home health services.
- Review and execute physician orders and ensure the plan of care reflects clients’ needs and goals.
- Educate clients and caregivers about diagnoses, medications, treatments, safety, and the home health plan of care.
- Communicate clinical findings and changes in condition to physicians and appropriate BAYADA care team members.
- Complete OASIS assessments and required admission documentation accurately and on time.
- Document assessments, interventions, education, and care coordination in BAYADA’s electronic medical record system.
- Support a smooth transition from referral to admission and ongoing home health services.
