About The Position

BAYADA Home Health Care is hiring an experienced, full-time Admissions Registered Nurse (RN) for its UCV office. The RN provides home health admissions and OASIS Start of Care (SOC) visits to adult and geriatric clients across Monmouth, Middlesex, Union, and Somerset Counties in New Jersey. The Admissions RN welcomes new clients and helps establish a successful home health experience. The role includes comprehensive initial nursing assessments, individualized care planning, and collaboration with an interdisciplinary team to support the transition into home health services. It offers one-on-one client care, autonomy in the field, flexible scheduling, per-point compensation, and opportunities for professional development. The position is focused on admissions and Start of Care rather than a traditional facility assignment. The RN works with one client at a time, tailoring care plans to each person's needs, goals, and home environment, while receiving support from an interdisciplinary team.

Requirements

Current and unrestricted New Jersey Registered Nurse (RN) license.
At least one year of recent, verifiable clinical nursing experience.
Prior home health experience.
OASIS experience.
Strong clinical assessment and care coordination skills.
Excellent communication, interpersonal, organizational, and time-management skills.
Ability to work independently and manage a field-based schedule effectively.
Reliable transportation and ability to travel throughout Monmouth, Middlesex, Union, and Somerset Counties.

Experience with electronic medical records (EMR).

Responsibilities

Complete comprehensive OASIS Start of Care (SOC) admissions visits for home health clients throughout the assigned territory.
Perform initial nursing assessments, including medication reconciliation, safety assessments, and evaluation of clients' clinical and functional needs.
Develop individualized plans of care in collaboration with physicians, clients, caregivers, and the interdisciplinary team.
Identify client needs and coordinate nursing, therapy, social work, and other home health services.
Review and execute physician orders and ensure care plans accurately reflect client needs and goals.
Educate clients and caregivers about diagnoses, medications, treatments, safety, and the home health plan of care.
Communicate clinical findings and changes in condition to physicians and appropriate BAYADA care team members.
Complete OASIS assessments and required admission documentation accurately and on time.
Document assessments, interventions, education, and care coordination in BAYADA's electronic medical record system.
Help ensure a smooth transition from referral to admission and ongoing home health services.
