About The Position
BAYADA Home Health Care is seeking an experienced Admissions Registered Nurse (RN) for a full-time position with its UCV office. The role provides home health admissions and OASIS Start of Care visits to adult and geriatric clients throughout Monmouth, Middlesex, Union, and Somerset Counties, New Jersey. The RN welcomes new clients, completes comprehensive initial assessments, establishes individualized plans of care, and collaborates with an interdisciplinary team to support clients’ transition into home health services. This autonomous, field-based position focuses on one client at a time and includes care coordination and communication with clients, families, physicians, and care team members. Compensation is listed as $65–$75 per point, depending on qualifications and status and subject to negotiation, with stated annual earning potential of $101,400–$117,000. A $20,000 sign-on bonus is offered to full-time RNs in Monmouth County only. The position includes flexible scheduling and a comprehensive benefits package. Requirements
- Current, unrestricted New Jersey Registered Nurse (RN) license.
- At least one year of recent, verifiable clinical nursing experience.
- Prior home health experience.
- OASIS experience.
- Strong clinical assessment and care coordination skills.
- Excellent communication, interpersonal, organizational, and time-management skills.
- Ability to work independently and manage a field-based schedule effectively.
- Reliable transportation and ability to travel throughout Monmouth, Middlesex, Union, and Somerset Counties.

- Experience with electronic medical records (EMR).
Responsibilities
- Complete comprehensive OASIS Start of Care admissions visits for home health clients throughout the assigned territory.
- Conduct initial nursing assessments, including medication reconciliation, safety assessments, and evaluation of clients’ clinical and functional needs.
- Develop individualized plans of care with physicians, clients, caregivers, and interdisciplinary team members.
- Identify client needs and coordinate nursing, therapy, social work, and other home health services.
- Review and execute physician orders and ensure plans of care reflect each client’s needs and goals.
- Educate clients and caregivers about diagnoses, medications, treatments, safety, and the home health plan of care.
- Communicate clinical findings and changes in condition to physicians and appropriate BAYADA care team members.
- Complete OASIS assessments and required admission documentation accurately and on time.
- Document assessments, interventions, education, and care coordination in BAYADA’s electronic medical record system.
- Support a smooth transition from referral to admission and ongoing home health services.
