About The Position

Select Home Health and Hospice is seeking a skilled and compassionate Admissions Registered Nurse to complete patient admission visits across the Indianapolis area. This floating position supports both home health and hospice services, ensuring patients receive thorough initial assessments, individualized plans of care, and a smooth transition into services.

The role involves completing admission visits and assessments, monitoring patient needs and changes in condition, coordinating care with physicians and the clinical team, documenting clinical information including OASIS assessments for eligible home health patients, and educating patients and caregivers. The nurse may provide additional in-home visits and nursing support based on agency needs, and will float throughout the Indianapolis area.

The position requires the ability to sit, stand, and drive for extended periods, assist and lift patients, and move equipment or supplies, with or without reasonable accommodation. A tablet is used regularly for documentation, communication, and training.

Requirements

• Graduate of an accredited school of professional nursing.
• Current Registered Nurse license for the practicing state of Indiana.
• Minimum of 1 year of nursing experience in a hospital, facility, or home health setting.
• Valid driver’s license, reliable auto insurance, and personal transportation.
• Current up-to-date TB test.

Responsibilities

• Complete patient admission visits and initial assessments for home health and hospice patients.
• Perform patient assessments and ongoing evaluations to monitor patient needs and changes in condition.
• Develop and implement individualized plans of care in collaboration with physicians and the clinical team.
• Complete required clinical documentation, including OASIS assessments for eligible home health patients.
• Provide in-home nursing care and additional patient visits as needed based on agency needs.
• Educate patients and caregivers on medications, diagnoses, treatment plans, and in-home care.
• Coordinate patient services and communicate changes in patient condition to physicians and the appropriate care team.
• Float throughout the Indianapolis area to provide admission coverage and additional nursing support as needed.
• Maintain compliance with applicable regulations, clinical standards, and agency policies.
