About The Position

Pomona Vista Care Center, a skilled nursing facility, is seeking a full-time Admissions Coordinator to join its clinical team and help provide residents with a memorable care experience. The role includes supporting facility activity programs and participating in resident and family planning, assessments, and discharge planning.

Requirements

• 1 year of experience in a Skilled Nursing Facility.
• 1 year of experience in an Admissions role.

Responsibilities

• Assist in planning, developing, organizing, implementing, and evaluating the facility’s activity programs.
• Observe resident attendance, mood, behavior, and involvement; note, report, and chart resident behavior to help evaluate activities and resident progress.
• Participate in community planning related to the facility’s interests and the services and needs of residents and families.
• Participate in discharge planning, development and implementation of activity care plans, and resident assessments.
• Interview residents or family members to obtain activity information.
• Involve residents and families in planning activity programs when possible.
• Involve residents and families in planning objectives and goals for residents.
• Assist in arranging transportation to other facilities when necessary.
