About The Position

Requirements

• Bachelor’s Degree required.
• 1-2 years of medical social work or case management experience preferred.
• LMSW or LCSW preferred.

Nice To Haves

• Master’s Degree preferred.

Responsibilities

• Provide emotional support, crisis intervention, advocacy, and bereavement services to patients and families.
• Document all interactions with patients and families in the electronic medical record.
• Facilitate the preadmission and admission process with the patient’s family.
• Act as a liaison between the interdisciplinary team and patient/family, facilitating communication, answering questions, and offering professional guidance.
• Ensure a safe discharge plan by collaborating with the patient/family and various disciplines.
• Follow up with patients and families after discharge to ensure satisfaction and continued support.
• Perform other duties as assigned.
