About The Position

Palo Verde Behavioral Health Hospital is seeking a compassionate, motivated, and Master's-level Social Worker to join our supportive Inpatient team. In this rewarding role, you'll work closely with patients, families, providers, and community partners to coordinate care, facilitate therapeutic services, and support individuals as they take important steps toward recovery. Palo Verde Behavioral Health is an 86-bed acute care behavioral health hospital serving adolescents and adults experiencing mental health and substance use challenges. We offer a full continuum of care, including inpatient, partial hospitalization, and intensive outpatient programs, and are proud of our reputation for delivering compassionate, high-quality behavioral healthcare. Requirements

  • Masters or Doctoral degree in Social Work, Psychology, Professional Counseling or related degree.
  • Experience coordinating care, familiarity with community resources, and ability to be self-organizing.
  • Must be certified in CPR upon hire and bi-annually thereafter.
  • Must possess or obtain and be able to perform Handle With Care within 30 days of hire and re-certify as needed.
  • Must complete all required mandatory in-services annually.
  • Must possess an Arizona Fingerprint Clearance Card or the ability to obtain.
  • Must be tested for Tuberculosis with a PPD skin test or chest x-ray upon hire and as required thereafter.

Nice To Haves

  • Experience in the field of Behavioral Health for at least 2 years is preferred, does not have to be obtained post-Master's

Responsibilities

  • Collaborate with referral sources, healthcare providers, and community partners to ensure seamless patient care.
  • Participate in daily interdisciplinary team meetings, coordinate follow-up appointments, facilitate Adult Recovery Team meetings, and support urgent patient needs.
  • Maintain regular communication with patients, families, and treatment professionals, ensuring everyone involved in the recovery process stays informed and supported.
  • Encourage patients to actively participate in their treatment journey, including group therapy, treatment planning, and discharge planning activities.
  • Connect patients with valuable community resources and support services that promote long-term wellness and successful recovery after discharge.
  • Adhere to program guidelines, treatment protocols, and HIPAA requirements while supporting the highest standards of patient confidentiality and care.
  • Complete excellent documentation to support patients in an appropriate length of stay and acceptance to referral Level of Care.
