About The Position

Requirements

• Bachelor’s Degree in Social Work or in Human Services
• 2 years of supervised social work experience in a health care setting working directly with individuals.
• Ability to read technical procedures.
• Ability to read and comprehend policy and procedure manuals.
• Ability to effectively present information and respond to questions from managers and employees.
• Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations.
• Ability to solve practical problems.
• Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

Nice To Haves

• MSW preferred
• Knowledge and experience with PCC preferred.

Responsibilities

• Establishes course of action by exploring options and setting goals with residents and their families.
• Assists residents in achieving the highest practicable level of self-care, independence, and well-being.
• Provides medically related social services to attain or maintain the highest practicable physical, mental, and psychosocial well-being of each resident.
• Evaluates social and family information, psychological and emotional needs to assess social services needs and develop care plans.
• Provides emotional support and addresses emotional problems, including assisting residents and families with coping mechanisms for physical disabilities, health-related fears, and grief.
• Assists residents with healthcare decisions.
• Assists with personal shopping.
• Assists in inventory and tracking of patient belongings.
• Coordinates response to reports of missing, lost, or stolen belongings.
• Assists in obtaining resources from community and social services agencies to meet resident needs.
• Assists in securing appropriate prosthetics and assistive devices.
• Develops and maintains strong working relationships with other departments and outside community agencies.
• Documents resident social service status.
• Assists in making outpatient appointments and scheduling on-site ancillary patient services.
• Assists in discharge planning with appropriate agencies, entities, or individuals.
• Coordinates with the interdisciplinary team.
• Provides in-service training to staff as requested.
• Counsels residents and family members.
