About The Position

Prava Therapy is seeking an entrepreneurial Physical Therapist or Occupational Therapist to lead and grow an on-site outpatient practice within two established senior living communities in Fresno, CA. This role offers the opportunity to build a concierge outpatient practice focused on relationship-based care for over 250 independent living residents. The practice owner will be responsible for providing movement and wellness expertise, helping residents stay independent, improve strength and balance, recover from surgery, prevent falls, and continue enjoying an active lifestyle. This is not a high-volume therapy model, but rather a relationship-focused approach delivered within the residents' living environment. Prava Therapy provides comprehensive support, including EMR and AI-powered documentation, billing and revenue cycle management, credentialing and contracting, marketing, resident education, referral development, operational support, and practice growth coaching.

Requirements

  • Licensed Physical Therapist or Occupational Therapist in California.
  • Entrepreneurial mindset.

Responsibilities

  • Lead and grow an on-site outpatient therapy practice within senior living communities.
  • Serve as the trusted movement and wellness expert for independent living residents.
  • Create meaningful relationships with residents and staff.
  • Develop and implement programs focused on resident independence, strength, balance, post-surgical recovery, fall prevention, and active lifestyles.
  • Deliver relationship-based care within the residents' living environment.
  • Focus on clinical excellence and practice growth.
  • Utilize EMR and AI-powered documentation tools.
  • Manage billing and revenue cycle.
  • Handle credentialing and contracting.
  • Engage in marketing and resident education initiatives.
  • Develop referrals.
  • Leverage operational support and practice growth coaching.
