About The Position

Requirements

• Experienced and able to provide care for all types of clients and all chronic conditions (companionship to Hospice Care).
• Pass all ABC exams for all levels of care.
• Able to drive to any location to provide care.
• Possess valid and current Drivers License and car insurance if applicable.
• Transport clients and co-workers in personal vehicle.
• Possess all state required documentation such as Nurse Aide school certificate and CPR/First Aide certificate. Documents must be current.

Responsibilities

• Implement individualized client care plans.
• Advocate patient welfare.
• Serve as a liaison between clients and their assigned care manager.
• Interact with Staffing Manager, Staffing Coordinators and Care Managers regarding assigned clients, unassigned clients, and their schedules.
• Cover last minute call outs for in-home care and staffing shifts in facilities.
• Transport clients and/or co-workers in personal vehicle or client’s vehicle.
• Service the entire island of Oahu.
• Work in all facilities ABC is contracted with.
• Cover shifts in facilities as needed.
