About The Position
STAT Home Health is seeking Full-Time Licensed Practical Nurses (LPN) in Houma, LA. This role offers a unique benefit of an Employee Stock Ownership Plan (ESOP), making employees true owners. The position involves providing nursing and supportive care, coordinating with physicians and other healthcare professionals, documenting services, and understanding Medicare/Medicaid regulations for home health.
Requirements
- Current Louisiana Licensed Practical Nurse (LPN) license required
- Minimum 1 year clinical experience as an LPN
- Current CPR Certification
- Compassionate and great time management skills
- Reliable transportation to conduct patient visits at their place of residence or facility
Responsibilities
- Provide nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN
- Communicate with the attending physicians, other home health team members, and other agencies as needed to coordinate optimal care and use of community resources for the patient/family.
- Document all services provided.
- Demonstrate knowledge of Medicare/Medicaid regulations for home health benefits.
- Report changes in patient condition and needs to RN
