About The Position

This is a full-time position for a Registered Dietitian at Advocate Trinity Hospital, working in a hospital-based diabetes clinic. The role involves providing comprehensive nutritional care to patients, including developing and implementing medical nutrition therapy, monitoring patient progress, and providing nutrition education to patients, families, and the public. The position also requires effective communication with healthcare professionals, mentoring students, and ensuring compliance with departmental standards and regulatory requirements. The dietitian will also participate in quality improvement programs and support system safety efforts.

Requirements

  • Graduate from a 4-year accredited college with a degree in Nutrition or Dietetics or equivalent.
  • Successful completion of The Academy of Nutrition and Dietetics registration requirements.
  • Excellent interpersonal and communication skills.
  • Ability to respond to the needs of a diverse patient population.
  • Ability to develop patient educational materials.
  • Registration by the Commission on Dietetic Registration.
  • Licensed by the State of Illinois.
  • Ability to be on your feet for extended periods of time.
  • Ability to lift up to 35 pounds without assistance.

Responsibilities

  • Provide nutritional care for patients in accordance with physicians' orders and standards of quality nutritional care, meeting age, psychosocial, and physical needs.
  • Review medical records for pertinent data, develop and implement medical nutrition therapy, and communicate with the health care team.
  • Monitor nutrition care, identify changes in patient nutritional status, and track progress.
  • Document nutrition interventions accurately and timely according to department standards and regulatory requirements.
  • Attend meetings, rounds, and care conferences to coordinate and collaborate on nutrition goals.
  • Order diets, supplements, nutrition support, vitamins/minerals, and labs as indicated within the scope of the MNT protocol.
  • Perform Nutrition Focused Physical Assessments on moderate and high nutrition risk patients.
  • Work with high-risk populations to prevent malnutrition and reduce readmissions.
  • Provide nutrition education on prescribed diets to inpatients, outpatients, family members, and the public through individual counseling, classes, and community education.
  • Evaluate and take appropriate action regarding nutrition consultations.
  • Provide nutrition education tailored to patient needs, readiness to learn, learning needs, and barriers to learning.
  • Document nutrition education accurately and timely.
  • Communicate effectively with patients/families, managers, supervisors, coordinators, peers, staff, and other health care professionals.
  • Mentor students, interns, and residents as assigned.
  • Serve as an educational preceptor to dietetic interns and nutrition students.
  • Follow HIPAA guidelines and established patient privacy rights.
  • Communicate according to Advocate's Mission, Values, and Philosophy guidelines.
  • Participate in interdisciplinary team meetings and other pertinent committees.
  • Serve as a liaison to physicians, house staff, nursing services, and food and nutrition services regarding clinical nutrition best practices.
  • Support department efforts to ensure compliance with standards, policies, and procedures, as well as DNV and local health department regulations.
  • Participate in the department's competency assessment program.
  • Participate in the department's PI program by accepting and completing assignments and working to evaluate processes.
  • Develop and revise nutrition education materials and departmental policies and procedures as requested.
  • Participate in the patient meal QA program and may provide operational support to the FNS department.
