About The Position

Requirements

• Excellent interpersonal skills such as communication, empathy, and compassion
• Demonstrated tact and patience
• Time management and organization skills
• High integrity and a commitment to HIPAA privacy standards
• Comfortable providing specialized care to the terminally ill
• At least 18 years of age
• Active Certified Nursing Assistant (CNA) designation current with state-specific requirements
• Licensed and insured driver with automobile in good working order

Responsibilities

• Provide patient care including bathing, nail/skin care, dressing/undressing, dental and other personal hygiene while maintaining a sanitary and orderly environment
• Offer positive, respectful, and confidential patient/family communication
• Take and record vital signs, and assist with self-administered medications
• Plan and prepare nutritious meals, including shopping and feeding the patient as needed
• Assess and report patient's condition and changes to the RN Case Manager
• Assist in ambulation, exercise, transfers, positioning and support a complete range of motion and other therapy activities
