About The Position
Gentiva is seeking a compassionate Hospice Chaplain to provide non-denominational spiritual and emotional support to patients and families receiving end-of-life care. The chaplain works as an integral member of the interdisciplinary team and provides inclusive care that respects patients’ beliefs, values, faiths, spiritual traditions, and cultural backgrounds. The role is listed as PRN, weekdays, dayshift, and is safety-sensitive. The chaplain assesses spiritual needs, develops individualized spiritual care plans, and provides requested visits, prayers, blessings, and rituals. The role also includes supporting families through grief, loss, or transition; coordinating with local faith leaders and community resources; collaborating with the care team; and maintaining timely, accurate documentation of spiritual assessments, interventions, and outcomes. The listed pay range is $28.00–$32.00 per hour, with compensation depending on factors such as location, experience, education, skills, licensure, certifications, and internal and department equity.
Requirements
- Meet one of the following educational requirements: a bachelor’s degree in theology, religion, human services, counseling, psychology, or sociology (or as required by state-specific regulations), or a minimum of one unit of ACPE-accredited Clinical Pastoral Education (CPE).
- If a degree is from a non-accredited college or university, AVPO approval is required before hire.
- Meet applicable state-specific requirements.

Responsibilities
- Provide spiritual and emotional support to patients, families, and caregivers in accordance with their beliefs and values.
- Provide non-denominational, inclusive care that respects all faiths, spiritual traditions, and cultural backgrounds.
- Participate in patient care conferences and collaborate with nurses, social workers, aides, and medical staff as part of the interdisciplinary team.
- Assess patients’ and families’ spiritual needs and develop individualized spiritual care plans.
- Conduct visits and provide prayers, blessings, and rituals when appropriate and requested by the patient or family.
- Assist families during grief, loss, or transition, and provide bereavement support and counseling.
- Build relationships with local faith leaders and community resources to support patients’ and families’ spiritual needs.
- Maintain timely and accurate documentation of spiritual assessments, interventions, and outcomes.
