About The Position

Hartford HealthCare at Home, the largest provider of homecare services in Connecticut, is seeking a Wound Care LPN to join their team. This role is part of a person-centered care model and will participate in the daily management and delivery of wound care for the Wound program. The LPN will aid in delivering outcome-based, cost-effective wound care by utilizing metric-driven decisions and collaborating with the Wound team RN and CWOCN to achieve exceptional outcomes. The position involves performing wound care, maintaining clinical care standards, and promoting daily operations and delivery of wound care. The LPN will also participate in ensuring consistent delivery of quality care, seeking consultation for complex patient problems, and performing both wound care and medical/surgical services. Communication with the Wound Team RN, CWOCN, and physician regarding changes in client needs and treatment plans is essential. The role also includes assisting with staffing needs, being accountable for team performance, staying updated on wound care trends, and participating in training and professional development.

Requirements

  • CT approved Licensed Practical Nursing program required.
  • Minimum of two years nursing experience in Clinical specialty area required.
  • Two years in homecare preferred.

Nice To Haves

  • Wound/Ostomy Assistant certification preferred.

Responsibilities

  • Participate in the daily management and delivery of wound care for the Wound program.
  • Aid in the delivery of outcome based wound care and cost-effective care by utilizing metric-driven decisions.
  • Collaborate with Wound team RN and CWOCN to achieve exceptional outcomes.
  • Perform wound care in the daily delivery of services and maintenance of clinical care standards for clients receiving specialized wound care treatments.
  • Promote the daily operations and delivery of wound care to patients.
  • Participate in the development, maintenance and implementation of program goals and objectives.
  • Ensure consistent delivery of quality care and communications.
  • Perform wound care to promote cost effectiveness for wound treatments in relationship to patient needs.
  • Seek consultation with the Wound Team RN and WOCN staff for patient problems/issues that require a change in the plan/interventions designed to solve complex problems involving client care.
  • Participate in the interdisciplinary approach to client care with Wound program/Agency staff.
  • Perform both Wound care and medical/surgical services Care required by the patient to achieve quality outcomes.
  • Inform the Wound Team RN, CWOCN and the physician/wound clinics of changes in client needs, wounds that require a change to the plan of care for wound care treatments and disease management.
  • Assist with staffing needs to meet service demands.
  • Be accountable for team performance in achieving desired clinical and operational performance measures.
  • Follow trends in specialty area of wounds for home care industry and make recommendations of treatment options for incorporation into the program.
  • Participate in training, coaching of staff or onboarding to the Wound program.
  • Participate and facilitate professional development needs and competency for wound program.
  • Actively participate in the performance improvement process known as H3W.
  • Participate in clinical education area to provide programs aimed to improve clinical competencies of direct service staff.
  • Research new information related to wound specialty and communicate to team.
  • Perform other duties as assigned.
