About The Position
The Wound Care LPN participates in the daily management and delivery of wound care for Hartford HealthCare at Home’s Wound program. The role provides outcome-based, cost-effective wound care using metric-driven decisions and collaborates with the Wound Team RN and CWOCN to achieve strong patient outcomes. The LPN performs wound care and medical/surgical services for clients receiving specialized treatments while maintaining clinical care standards. The position works with the Wound Team RN, CWOCN, and Manager of Specialty Services to support daily operations and the development, maintenance, and implementation of program goals. The LPN contributes to interdisciplinary client care, communicates changes in client needs to relevant clinical staff and physicians, supports staffing and staff development, and helps the team meet clinical and operational performance measures. The role also participates in performance improvement, clinical education, and research related to wound care.
Requirements
- Completion of a Connecticut-approved Licensed Practical Nursing program.
- At least two years of nursing experience in a clinical specialty area.
- Two years of home-care experience preferred.
Nice To Haves
- Wound/Ostomy Assistant certification.
- Two years of home-care experience.
Responsibilities
- Participate in the daily management and delivery of wound care for the Wound program.
- Provide outcome-based, cost-effective wound treatments in relation to patient needs.
- Perform wound care and medical/surgical services required by patients receiving specialized wound care.
- Consult with the Wound Team RN and WOCN staff about patient problems that may require changes to the plan or interventions.
- Participate in an interdisciplinary approach to client care with Wound program and agency staff.
- Inform the Wound Team RN, CWOCN, and physician or wound clinic of changes in client needs and wounds that require a change to the plan of care or disease-management approach.
- Assist with staffing needs to meet service demands.
- Support team performance in achieving clinical and operational performance measures.
- Monitor wound-care trends in the home-care industry and recommend treatment options for the program.
- Participate in staff training, coaching, and onboarding to the Wound program.
- Participate in professional development and competency activities for the Wound program.
- Participate in the H3W performance improvement process.
- Contribute to clinical education programs that improve direct-service staff competencies.
- Research new information related to wound specialty care and communicate it to the team.
- Perform other duties as assigned.
