About The Position

Masonicare Home Health & Hospice is seeking a Licensed Practical Nurse to provide skilled nursing care to ill or injured patients in their homes, hospitals, or extended care facilities. The LPN works under the direction of the Nurse Case Manager and/or Clinical Manager in home care cases, and under the direction of the institution’s shift supervisor when working in an institution. The role serves adult, geriatric, pediatric, and neonatal clients and supports the agency’s mission to provide excellent, affordable, and accessible care.

Requirements

Graduate of an NLN-accredited school of nursing.
Current State of Connecticut Practical Nurse license.
One year of acute care experience.
Current driver’s license.
Reliable transportation.

Nice To Haves

Previous homecare experience is strongly preferred.

Responsibilities

Coordinate care with community and agency resources and regularly with the designated registered nurse.
Receive assignments and reports for adult, geriatric, pediatric, and neonatal clients from a registered nurse or designee; accurately perform procedures and treatments and document them appropriately.
Communicate client care issues, observations, or concerns to the designated registered nurse or Clinical Manager in a timely manner, and coordinate care with the Nurse Case Manager.
Assist clients with socialization needs and promote education for clients, their families, and the community.
Enter patient and job information accurately and on time in the computer system to support service delivery and the preparation of time sheets and other required documents and reports.
Communicate effectively and tactfully with clients, taking into account their age, cultural diversity, needs, abilities, and physical condition.
Report to work on time, use time off appropriately, and complete annual education and medical requirements.
Perform other duties as assigned by the Clinical Manager or designee.
