About The Position

Patriot At Home is hiring a full-time Licensed Practical Nurse for homecare in Trumbull County. Business hours are Monday through Friday, 8:00 AM to 4:30 PM. The nurse assists the RN with carrying out and evaluating patient care plans, performing directed procedures, educating patients on self-care, preparing clinical progress notes, and supporting rehabilitation. The role involves visiting patients in their homes and completing standard nursing activities. Patriot At Home is veteran-owned and family-operated and provides Roger AI, an application designed to help complete home health documentation.

Requirements

• Graduation from an accredited School of Practical Nursing.
• Current LPN license in the State of Ohio.
• Criminal background check.
• Current CPR (BLS) certification.
• Valid driver's license.
• Ability to access patients' homes, which may not be wheelchair accessible.
• Sufficient hearing, eyesight, and physical dexterity for patient assessments and care.
• Ability to perform standard nursing activities, including walking, sitting, stooping, standing, and varying lifting, moving, and turning of patients.

Nice To Haves

• One or more years of home health experience is preferred.

Responsibilities

• Assist the RN in carrying out the plan of care.
• Assist the RN in performing specialized procedures as directed by the RN under physician's orders.
• Assist with the preparation, implementation, and continuing evaluation of the patient care plan.
• Help teach the patient appropriate self-care techniques.
• Prepare clinical progress notes.
• Assist with the rehabilitation of patients according to the medical care plan.
• Perform all other duties as assigned.
