LPN (Licensed Practical Nurse) - Homecare
About The Position
Patriot at Home is hiring a full-time Licensed Practical Nurse to provide homecare in Trumbull County. The LPN assists registered nurses in delivering and evaluating patient care, supporting patients with self-care and rehabilitation, and documenting clinical progress. The role may involve travel to patients’ homes, including homes that may not be wheelchair accessible. Business hours are Monday through Friday, 8:00 AM to 4:30 PM. The position offers a competitive salary, with the opportunity to earn additional money by picking up visits at a per diem rate. Requirements
- Graduation from an accredited School of Practical Nursing.
- Must be licensed in the State of Ohio as an LPN.
- Must have a criminal background check.
- Must have a current CPR (BLS) certification.
- Valid driver's license required.
- Ability to access patients' homes, which may not be wheelchair accessible.
- Sufficient hearing, eyesight, and physical dexterity for patient assessments and care.
- Ability to perform standard nursing activities, including walking, sitting, stooping, standing, and varying lifting, moving, and turning of patients.
Nice To Haves
- One or more years of home health experience is preferred.
Responsibilities
- Assist the RN in carrying out the plan of care.
- Assist the RN in performing specialized procedures as directed by the RN under physician's orders.
- Assist with the preparation, implementation, and continuing evaluation of the patient care plan.
- Help teach the patient appropriate self-care techniques.
- Prepare clinical progress notes.
- Assist with the rehabilitation of patients according to the medical care plan.
- Perform all other duties as assigned.
