About The Position

The Wellness Nurse (LPN/LVN) supports resident health at Sunrise Senior Living by monitoring care needs, promoting safe medication practices, and ensuring clinical compliance. The role includes conducting wellness assessments, monitoring changes in resident health, communicating with families, physicians, and care teams, and supporting infection control and clinical quality initiatives. The Wellness Nurse also helps train and support care team members.

Requirements

• Active LPN/LVN license.
• At least 1 year of experience in a senior living or clinical setting.
• Strong assessment and documentation skills.
• Knowledge of medication administration and compliance.
• Strong communication and problem-solving skills.

Responsibilities

• Conduct wellness assessments and monitor changes in resident health.
• Support medication management and perform MAR/TAR audits.
• Communicate with families, physicians, and care teams.
• Monitor incidents and falls and coordinate follow-up care.
• Maintain documentation and compliance with regulations.
• Support infection control and clinical quality initiatives.
• Train and support care team members.
