About The Position

At Sunrise Senior Living, the Wellness Nurse (LPN/LVN) supports resident health, monitors care needs, and ensures safe medication practices and clinical compliance. The role includes conducting wellness assessments, tracking health changes, coordinating with families, physicians, and care teams, and supporting clinical quality and infection control. Requirements

• Active LPN/LVN license.
• At least 1 year of experience in a senior living or clinical setting.
• Strong assessment and documentation skills.
• Knowledge of medication administration and compliance.
• Strong communication and problem-solving skills.

Responsibilities

• Conduct wellness assessments and monitor changes in residents’ health.
• Support medication management and perform MAR/TAR audits.
• Communicate with families, physicians, and care teams.
• Monitor incidents and falls and coordinate follow-up care.
• Maintain documentation and compliance with regulations.
• Support infection control and clinical quality initiatives.
• Train and support care team members.
