Wellness Nurse LPN LVN
About The Position
As a Wellness Nurse (LPN/LVN) at Sunrise Senior Living, you support resident health, monitor care needs, and help ensure safe medication practices and clinical compliance. You conduct wellness assessments, monitor health changes, support medication management, and work with families, physicians, and care teams. The role also involves monitoring incidents and falls, maintaining documentation, supporting infection control and clinical quality initiatives, and training and supporting care team members. Requirements
- Active LPN/LVN license.
- At least one year of experience in a senior living or clinical setting.
- Strong assessment and documentation skills.
- Knowledge of medication administration and compliance.
- Strong communication and problem-solving skills.
Responsibilities
- Conduct wellness assessments and monitor changes in residents’ health.
- Support medication management and perform MAR/TAR audits.
- Communicate with families, physicians, and care teams.
- Monitor incidents and falls and coordinate follow-up care.
- Maintain documentation and comply with regulations.
- Support infection control and clinical quality initiatives.
- Train and support care team members.
