About The Position

Sunrise Senior Living is hiring a Wellness Nurse (LPN/LVN) to support resident health, monitor care needs, ensure safe medication practices, and maintain clinical compliance. The role includes conducting wellness assessments, tracking changes in residents’ health, supporting medication management, and coordinating with families, physicians, and care teams. The nurse also monitors incidents and falls, supports follow-up care, maintains documentation, contributes to infection control and clinical quality initiatives, and trains and supports care team members.

Requirements

• Active LPN/LVN license.
• At least one year of experience in a senior living or clinical setting.
• Strong assessment and documentation skills.
• Knowledge of medication administration and compliance.
• Strong communication and problem-solving skills.

Responsibilities

• Conduct wellness assessments and monitor resident health changes.
• Support medication management and perform MAR/TAR audits.
• Communicate with families, physicians, and care teams.
• Monitor incidents and falls and coordinate follow-up care.
• Maintain documentation and comply with regulations.
• Support infection control and clinical quality initiatives.
• Train and support care team members.
