About The Position

Acts is seeking a Wellness Home Services Provider (CNA) to deliver compassionate, high-quality care to residents across all levels of care. The role provides direct, hands-on assistance with activities of daily living, including bathing, grooming, dressing, eating, toileting, and mobility, while prioritizing residents’ safety, comfort, and dignity. The provider monitors residents’ physical and emotional well-being and promptly reports changes to the appropriate supervisor to support proactive, personalized care.

Requirements

• Completion of an approved Certified Nursing Assistant (CNA) program, or equivalent verifiable training and experience that meets all applicable state requirements.
• Current CPR certification, or obtain certification within three months of employment.
• If responsible for transporting a resident, maintain a valid state driver’s license and provide proof of current automobile liability insurance.

Responsibilities

• Provide direct, hands-on assistance with daily living activities, including bathing, grooming, dressing, eating, toileting, and mobility.
• Prioritize resident safety, comfort, and dignity.
• Monitor residents’ physical and emotional well-being.
• Promptly report changes in residents’ condition to the appropriate supervisor.
