About The Position

Luminos Hospice is seeking a skilled and compassionate On-Call LPN to provide quality evening care to patients. The primary functions are to administer nursing care to patients of all ages in their places of residence and coordinate care with the interdisciplinary team, patients, families, and referring agencies. The LPN assumes responsibility for and participates in coordinating care.

The schedule is Monday through Thursday, 5 p.m. to 8 a.m.

Requirements

• Current state LPN license.
• Current driver’s license.
• Current CPR certification.
• Nursing skills consistent with generally accepted standards of practice.
• Knowledge of state and federal regulations.
• Ability to work independently and make sound decisions.
• Strong attention to detail and organizational habits.
• Excellent verbal and written communication skills.
• Good interpersonal skills.
• Reliable transportation and valid automobile liability insurance.

Nice To Haves

• Two years of experience as a nurse in a clinical care setting.
• One year of full-time hospice experience is preferred.

Responsibilities

• Administer nursing care to patients of all ages in their places of residence.
• Coordinate care with the interdisciplinary team, patients, families, and referring agencies.
• Assume responsibility for and participate in the coordination of care.
• Provide quality care to patients in the evenings.
