About The Position

Luminos Hospice is seeking a skilled and compassionate On-Call LPN to provide quality care to patients during weekends. The nurse administers care to patients of all ages in their place of residence, coordinates care with the Interdisciplinary Group/Team, patients, families, and referring agency, and participates in and assumes responsibility for care coordination.

Schedule: Friday 5pm–Monday 8am.

Requirements

• Current State LPN license.
• Current driver’s license.
• Current CPR Certification.
• Nursing skills consistent with generally accepted standards of practice.
• Knowledge of state and federal regulations.
• Ability to work independently and make sound decisions.
• Strong attention to detail and organizational habits.
• Excellent verbal and written communication skills.
• Good interpersonal skills.
• Reliable transportation and valid auto liability insurance.

Nice To Haves

• Two years' experience as a nurse in a clinical care setting.
• One year of full-time hospice experience.

Responsibilities

• Administer nursing care to patients of all ages in their place of residence.
• Coordinate care with the Interdisciplinary Group/Team, patients, families, and referring agency.
• Assume responsibility for and participate in care coordination.
• Provide quality care to patients during weekends.
